Hammertoes: Why Early Treatment Can Help Prevent Progression

Hammertoes develop when an imbalance in the muscles, tendons, and ligaments bends a toe at its middle joint. How Hammertoes Develop

Hammertoes most often affect the second, third, or fourth toes. Tight shoes, high heels, and narrow toe boxes can force the toes into a bent position. Bunions, flat feet, high arches, arthritis, previous injuries, and inherited foot structure may also contribute.
During the early stage, patients can often straighten the affected toe with their hands. As the condition progresses, shortened tendons and tightened joints may hold the toe in a fixed position. A rigid toe can cause persistent pain and make conservative treatment less effective.

Early Warning Signs to Recognize

Patients may first notice mild stiffness, cramping, or irritation while wearing certain shoes. The toe may begin to curl downward or rub against the top of the shoe. Repeated friction can create corns on the raised joint and calluses beneath the forefoot.
Without treatment, pain may increase during standing or walking. Patients may struggle to move the toe, find comfortable footwear, or maintain a normal stride. Changes in walking mechanics can also place additional pressure on nearby joints and other areas of the foot.

How Early Treatment Protects the Foot

We examine toe flexibility, joint position, foot structure, footwear, and walking mechanics. When hammertoes remain flexible, conservative care may ease symptoms and slow progression.
Treatment may include shoes with a wider and deeper toe box, protective padding, toe splints, or custom orthotics. Orthotics can redistribute pressure and improve alignment when abnormal mechanics contribute to the problem. Stretching and strengthening exercises may also improve muscle balance and maintain joint mobility.
We may carefully reduce painful corns or calluses when thickened skin adds discomfort. Patients should never cut these areas or use medicated corn removers without professional guidance, especially when diabetes, neuropathy, or poor circulation affects the feet.

A rigid, painful deformity may continue to interfere with walking despite changes in footwear and other conservative treatments.
